This Notice describes how health information about you may be used and disclosed and how you can get access to this information. If you are under 18 years of age, your parents or guardian must sign for you and handle your privacy rights for you. Please review it carefully.
Our Legal Duty
At Contour Solutions, we believe that your health information is personal. We keep records of the care and services that you receive at our facility private and secured. We are committed to keeping your health information private, and we are also required by the Health Insurance Portability and Accountability Act (HIPPA) to maintain and respect your confidentiality. We are also required to give you this Notice about our privacy practices, our legal duties, and your rights concerning your health information. We reserve the right to change our privacy practices and the terms of this Notice at any time, provided such changes are permitted by applicable law.
Acknowledgement of Receipt of this Notice
You will be asked to provide a signed acknowledgement of receipt of this notice. Our intent is to make you aware of the possible uses and disclosures of your protected health information and your privacy rights. If you decline to provide a signed acknowledgement, we will continue to provide your products and services, and will use and disclose your protected health information for treatment, payment, and health care operations when necessary.

When you become a patient of Contour Solutions, we will use your health information within Contour Solutions and disclose your health information outside Contour Solutions for the reasons described in this Notice. Health Care Services. We use your health information at Contour Solutions who need that information to assist your care. This may involve talking to doctors and others not employed by us. We also may disclose your health information to people outside Contour Solutions who may be involved in you health care, such as treating and/or referring doctors, home care providers, and family members.
Payment. We may use and disclose your health information so that the products and services you receive may be billed and paid for by you, your insurance company, or another third party. We may also tell your health plan a service you are going to receive so we can get prior payment approval or learn if your plan will pay for the products or services.
Health Care Operations. We may use your health information and disclose it outside Contour Solutions for our health care operations. These uses and disclosures help us operate Contour Solutions to maintain and improve patient care. We also may combine health information about many patients to identify new services to offers or what services are not needed. We may also disclose information to other persons at Contour Solutions for learning and quality improvement purposes.
Contacting You. We may use and disclose health information to reach you about appointments and other matters. We may contact you by mail, telephone or email. We may leave a voice message at the telephone number you provide us with, and we may respond to your email address. Marketing Health-Related Services. We will not use your health information for marketing communications without your written authorization.

We will not use or disclose your health information for other reasons without your written authorization. You may revoke the authorization, in writing, at any time, but we cannot take back any uses or disclosures of your health information already made with your authorization.
Abuse or Neglect
We may disclose your health information to appropriate authorities if we reasonably believe that you are a possible victim of abuse, neglect, or domestic violence or the possible victim of other crimes. We may disclose your health information to the extent necessary to avert a serious threat to your health or safety or the health or safety of others.
Patient Rights
Access: You have the right to look at or get copies of your health information, with limited exceptions. You may request that we provide copies in a format other than photocopies. We will use the format you request unless we cannot practicably do so. You must make a request, in writing, to obtain access to your health information. You may obtain a form to request access by sending us a letter to the address at the end of this notice. If you are requesting copies, there is a $15.00 copy fee (up to 30 pages) which includes staff time to locate and copy your health information, plus the cost of postage if you want the copies mailed to you. You have the right to receive an explanation of your account for the purposes of payment concerning your health care services.
Restriction
You have the right to request that we amend your health information. (Your request must be in writing, and it must explain why the information should be amended.) We may deny your request under certain circumstances.
Right to Request Confidential Communications. You have the right to request that we communicate with you about your health in a certain way or at a certain location. Your request for confidential communications must be in writing, signed and dated. It must identify the Contour Solutions or facility making the confidential communications and specify how or where you wish to be contacted. You need not to disclose the reason for your request. Right to a Paper Copy of This Notice. You have the right to a paper copy of this Notice. You may ask us to give you a copy of this Notice at any time. Questions and Complaints
If you believe your privacy rights have been violated, you may file a complaint with Contour Solutions or with the Secretary of the U.S. Department of Health and Human Services. To file a complaint with Contour Solutions, you must submit your complaint in writing to the Privacy Officer, Contour Solutions 8527 Village Dr., Ste 109 San Antonio, TX 78217. You will not be penalized for filing a complaint.
Changes To This Notice.
Contour Solutions may change this Notice at any time. Any changes in the Notice could apply to medical information we already have about you, as well as any information we receive in the future. We will post a copy of the current Notice at our facility and on our website, www.ContourSolutionsUSA.com. The effective date of the Notice is on the first page in the top left corner.

Your Personal Information
We at Contour Solutions recognize your right to confidentiality and are committed to protecting your privacy. We use the information that we collect on our site to provide you with a superior shopping experience. When you order, we will ask you to set up an account, which includes your name, e-mail address, mailing address, as well as certain other information that may be required to expedite your orders. In addition, we ask that you indicate whether you would like to receive e-mail from Contour Solutions keeping you informed of new products or features. If you would like to review or revise the information we have in your account, you may access such information by clicking on the "My Account" link on any screen.
We protect your account information against unauthorized access or release. We will not give, sell, rent, or loan any identifiable personal information to any third party, unless you have authorized us to or we are legally required to do so. We may share non-personal, summary, or aggregate customer data with partners and other third parties.
Security
When you place an order online, your personal information and credit card information are encrypted using SSL encryption technology before being sent over the Internet, making it virtually impossible for your information to be stolen or intercepted while being transferred to Contour Solutions. While on a secure page, such as our order form, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to un-locked, or open, when you are just "surfing".

Return Policy
Thank you for your recent purchase! If for any reason you would like to return or exchange your product(s), we will be glad to exchange it for another size or refund your money. Please call us at 1-888-946-6004 within 10 days of receipt of your product(s) and we will provide you with a return authorization number and return instructions for shipping if the following criteria is met below.
All exchanges and/or returns must meet the following criteria (No Exceptions):
- Exchanges or returns must be done within 10 business days. No exceptions.
- Products must be returned in its original condition and its original packaging.
- Products that smell of smoke, have the tags removed, have been worn, or laundered cannot be returned and will not be refunded. (Any items mailed or left at our store without an RA# will be refused and or discarded.)
- Personal items such as panties, underwear, headwear and swimwear can not be returned due to health regulations.
- Packaged containers must not have any markings or personal labels, of any manner.
- All MADE-TO-ORDER OR CUSTOM ORDERED Compression Garments, JODEE Perma-Form Bras, Trulife Customer Breast Forms are NON-RETURNABLE AND NON-EXCHANGEABLE.
Exchange for different size/color:
If your items meet the following criteria for an exchange, please mail a check or money order for $6.50 to cover shipping/handling charge of the exchanged item to:
along with the form below indicating your desired selection. If you need assistance, please call us Monday – Thursday (8:30 – 5:30 CST) at 1-888-946-6004. Our after-hours answering service is unable to assist with your orders but feel free to leave a message and we will call you back the next business day.
Damaged or Manufacture Defect Merchandise:
If you received a damaged item or you feel the item may be defective, please contact us immediately (within 3 days of receiving your delivery). Replacement of a defective item will be evaluated by the manufacturer and replaced if product is found to be defective. Refund or exchange of a damaged item will be subject to a manufacture evaluation.
Refunds (no exchanges):
If your order meets our return criteria, then we will gladly refund your merchandise for the item(s), (less the shipping/handling). If you do not exchange your item, a 15% restocking fee of the returned item(s) will be charged to your credit card or deducted to from your return credit. Please fill out the bottom portion of this form and return it with your products within 10 business days.
Shipping Policy
Free Shipping Over $50:
Free shipping for orders valued over $50 after discount in the continental USA only. Shipping charges apply to orders shipped to Hawaii, Alaska and other outlying USA territories, Canada, Europe, Asia and international.
Shipping USA Orders:
Continental USA: Free shipping for orders over $50 shipped UPS. If order is over $50 please provide a physical shipping address for UPS delivery. Additional shipping charges may apply for P.O. Box, Alaska, Hawaii, and outlying USA territories.
We ship to APO addresses via UPS.
International Orders:
Please note that the international shipping and billing address are to be the same. For first time international customers that order over $50, proof of card ownership is to be emailed before order will be shipped. Please email a copy of the front and back of your card with a copy of a statement that reflects our charge. For security remove / blank out the first 10 numbers of the credit card and account number. You may also remove other charges on the statement (we only need to see ours).
You may be charged import taxes, duties and VAT by your local government. These vary in every country. We suggest you contact your nearest post office to confirm any taxes that you may be charged for overseas posted items.
The online shipping calculation is an estimate and additional shipping charges may apply.

Certain US orders and International Orders may require additional shipping charges. If your ship to address requires additional shipping charges we will notify you of these additional charges, via email.

Signature Delivery on Shipments
In order to prevent the loss, theft or damage of your order, a signature is required upon delivery by the carrier. By waiving this option, the recipient becomes solely responsible for the shipment once it has been dropped off by the carrier. Under these circumstances, lost, stolen or otherwise damaged goods will not be reimbursed by either Contour Solutions or its designated carrier.
Shipping Disclaimer:
Please allow 1-3 Business days for order processing. Orders placed the same day before 11:30am CST for Expedited delivery only will be shipped the same day. Shipping services may be downgraded if shipper determines that order will arrive in the same time by downgraded service. If this event occurs, your account will be charged according to the method of shipment. Large or overweight items may be shipped separately by common carrier at Contour Solutions discretion. Items ordered are subject to availability. If an event occurs where an item is out of stock, you will be notified by email or telephone.
